Latest news for Women Deliver

Women Deliver Maternal Health theme: Call for Topics

Petra ten Hoope-Bender, the Institute’s Director for Reproductive, Maternal, Newborn and Child Health (RMNCH), is the lead organizer for the Maternal and Newborn Health Theme at the 2013 Women Deliver Conference. Together with Jeff Smith from Jhpiego, Mary Nell Wegner from the Maternal Health Task Force and Winnie Mwebesa and Claudia Morrissey from Save the Children, Petra will be putting together 12 panel sessions on the hot topics in the area of maternal and newborn health. Some of the ideas already under consideration are morbidities, young motherhood, midwifery and m-health. Women Deliver is expecting some 5000 participants at the conference in Kuala Lumpur, 28-30 May 2013.

Please read this invitation which will be sent to a wide international audience and feel free to forward it to colleagues so that we reach as broad a group of individuals and organisations as possible. The closing date for topic suggestions is 14 September 2012.

What Makes A Good Clinical Instructor?
It is easy to find individuals who want to be IBCLCs. In many parts of the world, there are programs in a variety of formats, styles, and locations that teach the essential didactic background of lactation consulting. What is missing are sufficient numbers of qualified individuals who will offer supervised clinical instruction for the preparation of IBCLCs.
Clinical instructors require a special skill set that enables them to guide new professionals in the acquisition of knowledge and skills. Not all experienced IBCLCs want to serve as clinical instructors. Of those who wish to do so, not all will have the necessary teaching skills, experience, or professional and personal attributes that enable them to be effective clinical instructors. This chapter will explore what all good clinical instructors have in common and what can make them great.
Definition of a Clinical Instructor
The words “mentor” and “preceptor” sometimes are used interchangeably to describe individuals who provide expertise and experience and act as a role model. Figure 2.1 demonstrates how the meaning of each differs subtly from one another and sets them apart from the definition of a clinical instructor. The term “clinical instructor” as used in this text describes the person who provides educational preparation to the aspiring lactation consultant.

มหัศจรรย์นมแม่








ถ้าเด็กทารกทุกคนบนโลกได้กินนมแม่ คงจะมีเด็กที่ป่วยน้อยลงมากเพราะ
มีงานวิจัยออกมารองรับมากมายว่า นมแม่เป็นอาหารมหัศจรรย์สำหรับเด็กทารก
ทุกคน ดังนั้น เมื่อคุณแม่มีลูกของขวัญที่ดีที่สุดที่คุณจะมอบให้กับลูกได้ก็คือ
 นมแม่จากอกแม่นั่นเอง

มีหลายๆท่านที่ให้นมแม่ไม่สำเร็จด้วยสาเหตุต่างๆ ถ้าเป็นการเจ็บป่วยทางร่างกายก็เป็น
ข้อยกเว้นจริงๆ แต่หลายคนที่ให้นมแม่ไม่ได้อาจจะเกิดจากการรู้เท่าไม่ถึงการไม่รู้ถึง
เทคนิค และวิธีการให้นมแม่อย่างถูกต้อง จึงทำให้เกิดความเครียด ท้อถอย นมแม่ 
ก็แห้งไปในที่สุด

ดูดนม ต้องดูดให้ถูกวิธี

เริ่มตั้งแต่วิธีให้ลูกอมหัวนมแม่
การดูดให้ถูกวิธีจะทำให้ลูกได้รับน้ำนมแม่อย่างเพียงพอ นมแม่ได้รับการกระตุ้นที่

ถูกวิธีจะทำให้มีการผลิตน้ำนมอย่างสมำเสมอ ซึ่งจะต้องเริ่มจากการอมหัวนมของ
ลูกน้อย เขี่ยที่ริมฝีปากล่างลูกเบาๆ ลูกจะอ้าปาก ลิ้นเคลื่อนออกมาเล็กน้อย ให้อุ้ม
ลูกเข้าหาหัวนมแม่โดยให้อมจนเต็มลานหัวนม ไม่ต้องกลัวว่าจะคับปากลูก เพราะ
หัวนมและลานหัวนมจะลู่ไปตามช่องปากของลูก เหงือกจะงับอยู่บนลานหัวนม 
ลิ้นจะอยู่ใต้ลานหัวนม ห่อหัวนม และลานหัวนม พร้อมกับดันขึ้นไปแนบกับเพดานปาก
 ขณะดูดจะเกิดการหดตัวของกล้ามเนื้อ ลิ้นจากปลายจนถึงโคนลิ้น เหมือนลูกคลื่น 
เป็นการรีดนม ส่วนเหงือกที่งับบนลานหัวนมที่มีกระเปาะน้ำนมอยู่จะงับเข้าหากันเป็น
จังหวะ ทำให้น้ำนมพุ่งเข้าปาก
ส่วนการดูดนมขวด เพียงแต่คว่ำขวดลง นมก้ไหลเข้าปากลูกแล้ว ลูกไม่จำเป็นต้อง
ใช้สิ้นและเหงือก ง่ายกว่าการดูดนมจากอกแม่มาก นี่จึงเป็นเหตุผลว่าทำไมเด็กแรกเกิด
ไม่ควรดูดนมจากขวดตั้งแต่แรกเกิด

ท่าให้นมลูกใครว่าไม่สำคัญ
ท่าให้นมลูกเป็นส่วนสำคัญอีกอย่างที่จะทำให้คุณแม่ประสบความสำเร็จในการให้นมลูกอย่างยิ่ง



1.ท่านอนเปล คุณแม่ที่หายเจ็บแผลหรือหายอ่อนเพลียจากการคลอดลูกแล้ว มักเลือกใช้ท่านี้เป็นส่วนใหญ่
*นั่งบนเก้าอี้หรือเตียงนอนที่สามารถพงหลังได้อย่างสบาย
*อุ้มลูกอยู่ในอ้อมแขน ให้ศีรษะลูกอยู่ในช่วงเว้าของข้อศอก ส่วนแขนประคองลำตัวลูกโดยที่ฝ่ามือหรือบริเวณสะโพก หรือก้นของลูก
*หน้าลูกจะหันเข้าหาเต้านมแม่โดยอัตโนมัติ
*มืออีกข้างอาจจะช่วยประคองก้นลูก หรือประคองเต้านมข้างที่ลูกดูดเพื่อให้หัวนมเข้าปากลูกให้ลึกที่สุด

2.ท่าเปลขวาง ท่านี้จะคล้ายกับท่าที่ 1 เพียงแต่ใช้มือสลับข้างกัน
*ใช้มือด้านตรงข้ามกับเต้านมที่ให้ลูกดูดนม ประคองหัวและคอของลูกเอาไว้
*เป็นท่าที่เหมาะสำหรับจะถอนหัวนมออกจากปากของลูกโดยการใช้นิ้วก้อยสอดเข้าไประหว่างเหงือกกับหัวนม หรือจับตรงปลายคาง ให้ปากลูกอ้าออก
*ใช้ท่านี้สลับกับท่าที่ 1 เวลาที่ลูกดิ้นเพราะอึดอัด หรืออยากจะเปลี่ยนท่า

3.อุ้มลูกรักบี้ เหมาะสำหรับคุณแม่ที่ผ่าตัดคลอดและยังเจ็บแผลอยู่ การใช้ท่านี้ลูกไม่ต้องนอนบนตัวคุณแม่ และเหมาะสำหรับคุณแม่ลูกแฝดที่ให้นมลูกพร้อมกัน
*หาที่นั่งสบายที่มีพนักพิง
*อุ้มลูกไว้ข้างลำตัว ขาลูกจะอยู่ใต้วงแขนของคุณแม่
*ช่วงลำตัวลูกจะอยู่บนแขนคุณแม่ และส่วนศีรษะลูกจะอยู่บนฝ่ามือขยับตัวและศีรษะลูกให้พอดีกับเต้านม
*ใช้มืออีกข้างช่วยประคองเต้านม เพื่อให้ลูกดูดได้สะดวกขึ้น

4.ท่านอน ท่านี้คุณแม่ทั้งหลายชอบ เพราะไม่ทำให้เจ็บแผล ไม่ต้องขยับตัวมาก
*คุณแม่นอนตะแคงข้าง อาจจะหาหมอนมาหนุนบริเวณหัวไหล่
*ให้ลูกนอนตะแคงหันหน้าเข้าหาแม่ โดยจัดให้ปากของลูกงับหัวนมแม่ได้พอดี
*ถ้าจะเปลี่ยนเต้าให้ลูกดูด คุณแม่ก็เพียงแต่นอนกึ่งๆเหมือนนอนคว่ำ ให้เต้านมแม่โน้มลงมาพอดีกับปากลูก
*สิ่งที่ต้องระวังคือ อย่าให้หน้าอกคุณแม่ทับจมูกลูกโดยเฉพาะเวลากลางคืน คุณแม่ต้องระวังจะเผลอหลับไปก่อนที่ลูกจะดูดนมเสร็จ
